Who we are...
CCCC is a youth and family driven non-profit care management organization (CMO) supporting young people up to age 21 who may be facing emotional, behavioral, or substance use challenges, have developmental disabilities, or are involved in the juvenile justice system. By focusing on each child’s strengths and surrounding them with a caring team, we work to help them stay safe, supported, and thriving at home, in school, and in their community.
What we do...
CCCC works alongside children, teens, and their families to create a plan for success that reflects their own goals and hopes for the future. Together, we explore strengths, needs, and helpful supports to build an Individualized Service Plan (ISP) that truly fits each family. With the help of both personal and community supports, we form a Child and Family Team (CFT) that connects families to long term resources so they can grow, succeed, and thrive independently.
