Care Manager

To facilitate positive change in children with emotional and behavioral challenges along with their families, following the Wraparound process and the strength-based philosophy in partnership with stakeholders and service providers in the NJ System of Care network.

Requirements:

Work with children, families, providers & the community in Mercer County, New Jersey, to coordinate care in multiple systems.

• Bachelor's degree & 1 year experience in a related position, preferably with at-risk children & families
• Prior experience in managing wraparound/individual services. Developmental Disabilities and/or Substance Abuse experience a plus
• Valid driver's license required
• Excellent communication (verbal and written), interpersonal, organizational, time management and assessment skills
• Possess energy, creativity, and cultural competence.
• Able to work under supervision and as part of a team
• Bilingual language skills (Spanish or other language(s)) in addition to English, a plus
• MSW desirable (with chance to advance!)
• Daily work hours may vary depending on the needs of the families served. In addition, must be able to work a flexible schedule to accommodate families’ needs when it comes to scheduling visits.
